Applying for a United States passport?
Benton County Records and Elections Department is a passport acceptance agency. Call us at 541-766-6756 to make an appointment and with any questions. Please note that we will not accept any passport appointments from May 13, 2026 through May 20, 2026 due to the Primary Election.
Passport Fees and Forms of Payment
- Payment to US Dept. of State must be personal check, money order, bank draft, or US cashier’s check.
- Payment to Benton County can be personal check, money order, bank draft, US cashier’s check, debit or credit card. Card charges a 2.49% third party fee.
- We take passport photos for $20.00.
Age 18 & Over
First
Passport
Applicant must submit application in person with the items below:
- Completed form DS-11.
- Proof of citizenship.
- Current valid photo identification. If current valid ID is an out of state ID, Provisional Drivers’ License, State ID or Driving Permit, bring secondary valid identification.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
Age 18 & Over
Passport Renewal
Applicant must use either Form DS-82 or Form DS-11. Read carefully which form to use.
- Use Form DS-82 if you can answer “Yes” to all the statements. You need not appear at our office if you can renew by mail with Form DS-82.
- If unable to renew by mail, complete Form DS-11 and apply in person at our office with the items below:
- Completed Form DS-11.
- Most recent passport.
- If you have lost your current unexpired passport, bring completed Form DS-64 and bring proof of citizenship.
- Current valid photo identification. If current valid ID is an out of state ID, Provisional Drivers’ License, State ID or Driving Permit, bring secondary valid identification.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
Ages 16 & 17
First Passport
& Renewal
Applicant and one parent must be present with the items below:
- Completed form DS-11.
- Proof of citizenship.
- Most recent passport.
- If you have lost your current unexpired passport, bring completed Form DS-64 and bring proof of citizenship.
- Current valid photo identification. If current valid ID is an out of state ID, Provisional Drivers’ License, State ID or Driving Permit, bring secondary valid identification.
- Parent’s current valid photo identification.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
Age 15 & Under
First Passport
& Renewal
Minor must be present with both parents. Bring the following:
- Completed form DS-11.
- Proof of citizenship.
- Most recent passport.
- If you have lost your current unexpired passport, bring completed Form DS-64 and bring proof of citizenship.
- Proof of parental relationship such as birth certificate.
- Parents’ valid photo identifications.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
