Getting a Passport in Benton County
Benton County Records and Elections Department is a passport acceptance agency.

Passport Fees and Forms of Payment
- Payment to U.S. Department of State must be personal check, money order, bank draft or U.S. cashier’s check.
- Payment to Benton County can be personal check, money order, bank draft, US cashier’s check, debit or credit card. Card charges a 2.49% convenience fee.
- We take passport photos for $20.00.

Requirement overview
Age 18+, First Passport
Apply for Your First Passport for ages 18+
- Completed form DS-11.
- Proof of citizenship.
- Current valid photo identification. If current valid ID is an out of state ID, Provisional Drivers’ License, State ID or Driving Permit, bring secondary valid identification.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
Age 18+, Passport Renewal
Renew Your Passport for ages 18+
Applicant must use either Form DS-82 or Form DS-11. Read carefully which form to use.
- Use Form DS-82 if you can answer “Yes” to all the statements. You need not appear at our office if you can renew by mail with Form DS-82.
- If you have do not currently possess your passport (lost or stolen), explain the situation in detail in Section 21 on the DS-11 form.
- If unable to renew by mail, complete Form DS-11 and apply in person at our office with the items below:
- Completed Form DS-11.
- Most recent passport.
- Current valid photo identification. If current valid ID is an out of state ID, Provisional Drivers’ License, State ID or Driving Permit, bring secondary valid identification.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
Age 16 & 17, First Passport OR Renewal
Apply for or Renew a Passport for ages 16 & 17
Applicant and one parent must be present with the items below:
- Completed form DS-11.
- Proof of citizenship.
- Most recent passport.
- If you have do not currently possess your passport (lost or stolen), explain the situation in detail in Section 21 on the DS-11 form.
- Current valid photo identification. If current valid ID is an out of state ID, Provisional Drivers’ License, State ID or Driving Permit, bring secondary valid identification.
- Parent’s current valid photo identification.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
Age 15 & Under, First Passport OR Renewal
Apply for or Renew a Passport for ages 15 & under
Minor must be present with both parents. Bring the following:
- Completed form DS-11.
- Proof of citizenship.
- Most recent passport.
- If you have do not currently possess your passport (lost or stolen), explain the situation in detail in Section 21 on the DS-11 form.
- Proof of parental relationship such as birth certificate.
- Parents’ valid photo identifications.
- Passport photo. Can be taken here for $20.00.
- Two separate payments.
